Our time will be dedicated to facilitating the rehabilitation of the client through expert planning, coordination, rehabilitation along with care and support. We aim to assist the client’s access to the community to the best of our ability allowing them to enjoy the pleasures of day to day life. Our mission statement is to provide the highest quality of life possible to those suffering from Brain Injuries.
Spotlight will discuss with you and the client where appropriate, their needs and co-ordinate a bespoke package of rehabilitation and care.
Our Case Management service includes:
- Immediate Needs Assessment, compiling a comprehensive report with recommendations and costings
- Liaison with solicitors and appointed deputies involved in the litigation and Court of Protection (COP) processes
- Comprehensive rehabilitation care planning including detailed risk assessments
- Planning and implementing client specific goal management
- Ongoing co-ordination and management of bespoke rehabilitation, care and support
- Recruitment, induction, training and support for support worker staff teams
- Assistance with researching and securing provision of therapeutic services
- Support with implementation, review or transitions of/to an Education and Health Care Plan (EHCP)
- Attending appointments and meetings with or/on behalf of the client/family
- Organisation of multi-disciplinary meetings to ensure a seamless approach across services
- Sourcing appropriate equipment/vehicles to meet your client’s needs
- Sourcing appropriate accommodation to meet your client’s needs
- Support with accessing benefits and specific financial issues
- Advocacy for the client and their family
